Claims submitted to the Carrier via the Kiosk

If denied, the
employee must print the claim out and forward to his/her Local Chairman for
further handling if desired.

If penalty claim
is handwritten, it must be faxed to IHB extension 6723. If denied, penalty time claim will be
returned to the employee via
We encourage employees to use
the kiosk method for claims as this is the most direct and expedient.
Any penalty claims received through other than the
above methods will be regarded as not received. Be governed
accordingly.
