EMPLOYEE PRESCRIPTION DRUG REPORT

Employees using any prescription, drug, medication, or controlled substance must submit an Employee Prescription Drug Report. This report must be completed by the prescribing physician and submitted to the IHB Medical Director for review and

approval.
