ACTIVITY REPORT

The taxi portion of the activity report must be properly completed with all five lines documented:

All the above information is required to allow the carrier to monitor taxi expenditures.

As previously instructed, transportation crews must arrange to fax their activity reports to the data input department at Gibson Ext. 4932 promptly at the end of their tour.

Arrange to review the activity report prior to faxing to ensure that all required information has been documented in all sections:

Incomplete activity reports will be returned for required information in order to effectively manage expenditures and payments. The information must flow accurately and timely.
